Shepherd's Heart Counseling is based in Siler City and works with families across Chatham County and its neighboring communities. This page is about what care here actually looks like in a rural North Carolina county — who calls, what they're bringing in, and how privacy and access work in a place this size. ## A rural county with a big-county problem. Chatham County stretches from the edges of the Triangle down through Pittsboro and out to [Siler City](/locations/siler-city), Bear Creek, Bennett, Goldston, and the smaller communities that sit between them. It's a county where families still drive twenty or thirty minutes for groceries and forty-five for a specialist. Mental-health care that focuses on parents, teens, and families is not evenly distributed here, and most of what does exist is concentrated closer to Chapel Hill. Shepherd's Heart Counseling sits deliberately on the western side of the county so that families in and around Siler City don't have to drive across the county line to see someone. ## The small-town confidentiality question. In a small county, everyone knows someone who knows someone. Your child's teacher goes to your church. The receptionist at your pediatrician is your neighbor's cousin. That is one of the reasons families here quietly avoid getting help — the fear that seeking counseling will end up as a piece of information moving around the community. This is not a paranoid concern; it's an accurate one for a place this size. Confidentiality is treated correspondingly seriously here. Nobody sees your record without your explicit consent. The [in-person](/locations) clients most often come from Siler City itself, from Pittsboro, and from the smaller communities across the western side of the county — Bear Creek, Bennett, Goldston, Bonlee, Snow Camp. A number come across county lines from Liberty, Ramseur, and as far as Asheboro when they want a specialist who works with parents and teens and haven't found one closer. For clients further out, telehealth removes the drive entirely — same clinician, same fee, same insurance relationships. ## What we work on here. The reasons families in this part of North Carolina call are pretty consistent. Parents worn down by a child whose behavior is running the house. Teens who have gone flat or gone quiet. Marriages fraying under years of caregiving. Educators, first responders, and DSS staff carrying home the weight of a job most of their neighbors underestimate. Whole families where the conflict has become the default setting. All of that fits inside the scope of the practice. Where a case needs something more specialized — psychiatry, medication management, formal ADHD or autism testing — we say so and help you find the right referral in the region. ## What Chatham County families should know before calling. The practice takes Aetna and Blue Cross Blue Shield of North Carolina in network. It does not take Medicaid or Medicare. Sessions are $155 and intake is $195 for anyone paying out of pocket. Consultations are 15 minutes and free. Therapy is only available to clients physically located in North Carolina at the time of session — that's a licensure rule, not a preference, and it applies whether the session is in the office or on video.
