# Psychotherapy in Siler City, NC

## Understanding a pattern vs. managing it.

Individual therapy on this site is generally focused: you have anxiety, or a hard season, or a specific problem, and we work on it until you don't. Psychotherapy is a different frame. It's for the client who has already done some of that work, or who suspects they'll be back inside a year if they only work on the surface, and who wants to understand where the pattern actually comes from. Why do I always end up in the same fight with the same kind of person. Why does my competence at work never feel real. Why do I disappear when someone gets close. The question isn't 'how do I cope with this.' The question is 'why is this here at all.'

## How this compares to individual therapy here.

Same clinician, same office, same session. Different orientation. Individual therapy is usually more directive, more structured around a problem, and time-bounded. Psychotherapy is more open-ended, less agenda-driven, and moves at the pace of the material rather than at the pace of a treatment plan. Some clients start in individual therapy and move over to psychotherapy when they realize the thing they came in with is a symptom of something older. Others start in psychotherapy from the beginning because they already know that.

## The methods, and why the room feels different.

Psychodynamic work is the spine — the assumption that early relationships and the way you were treated laid patterns that are still running underneath your adult life. Woven in: cognitive work when a pattern of thinking needs to be interrupted, somatic and mindfulness practices when the pattern is stored in the body, and EMDR when a specific traumatic memory is driving something we're working on (John is trained in EMDR, not certified). Sessions are more unhurried than in the other services. There is more silence, more room for something to surface that neither of us saw coming, and less pressure to arrive at a clear takeaway by the end of the hour.

## It takes longer, and it asks more of you.

This is worth naming directly. Psychotherapy is typically a multi-year arrangement. Weekly sessions are the norm; bi-weekly slows the work meaningfully. Progress rarely looks like a checkbox — it looks like a version of you that behaves differently in a situation that used to trigger the same old reflex, and often you notice the change before anyone else does. It requires enough emotional bandwidth to keep showing up when the material is uncomfortable, enough stability in the rest of your life to hold what surfaces between sessions, and a willingness to sit with not-knowing. If you are in acute crisis, or if your basic conditions (housing, income, safety) are not stable, this is usually not the right service to start with.

## Who this fits.

Adults who have done some prior therapy and want to go deeper, or who are new to therapy but come in already asking why-questions rather than how-questions. People with recurring relationship patterns they can name but can't seem to change. Adult children of dysfunctional families who are, on paper, doing fine but who feel a persistent gap between the life they've built and the person they experience themselves as being. Parents who have realized the way they parent is being pulled by material from their own childhood and who want to work with the source, not just the symptom.

## Practical details.

Fees are $195 intake, $155 per session. Shepherd's Heart is in-network with Aetna and Blue Cross Blue Shield of North Carolina; a Good Faith Estimate is provided at intake. Weekly is the default cadence for depth work. Sessions are in the [Siler City office](/locations/siler-city) or by [telehealth](/telehealth-north-carolina) anywhere in NC. Care is provided by John C. Murphy, MA, LCMHC (NC #15133).
